Sinus pause bradycardia and bilious vomiting with associated dizziness: case report A 26-year-old woman developed sinus pause bradycardia, and bilious vomiting with associated dizziness during treatment with irinotecan for Ewing’s-like sarcoma with metastasis to the right and left lung. The woman, who had Ewing’s-like sarcoma and metastasis to the right and left lung after resection of the left lower lobe, started receiving treatment with irinotecan [dosage not stated] along with temozolomide. Post chemotherapy, she developed adverse effect of irinotecan in the form of bilious vomiting with associated dizziness. The woman’s adverse effect was managed with lorazepam [Ativan] and ondansetron. She experienced occasional lightheadedness and decreased fluid intake by mouth. Examination revealed hypotension and bradycardia. She was alert, oriented and awake. ECG showed bradycardia with sinus pauses of less than a second. Based on these findings, a diagnosis of sinus pause bradycardia associated with irinotecan was confirmed [duration of treatment to reaction onset not stated]. Subsequently, she was treated with atropine, and her bradycardia resolved. She completed her chemotherapy with no other complaints. Mir TA, et al.
