Methylprednisolone Herpes simplex virus type 2 infection following intra-articular injection: case report A 51-year-old woman with sacroiliitis received an intraarticular sacroiliac joint injection with methylprednisolone 80mg. After 8 days, she reported that she had noted a pruritic rash near the injection site 2 days after the injection. She reported taking paracetamol [acetaminophen], which relieved her symptoms. She also reported that she had had chickenpox as a child. Physical examination revealed a cluster of vesicles in her right medial buttock, which was located 1cm inferior to the injection site. The affected area was mildly tender on palpation. Viral analysis by PCR of a swab of the vesicle contents was positive for herpes simplex virus (HSV) type 2. She received topical aciclovir and her eruption responded well. Author comment: "[T]he occurrence of the eruption shortly following the injection and the location of the eruption adjacent to the site of injection imply a causal link between the intra-articular corticosteroid injection and the eruption, rather than just coincidental infection."
