Treatment failure: case report A 72-year-old woman exhibited treatment failure while being treated with methylprednisolone for Hashimoto’s encephalopathy (HE). The woman, who had a history of hypertension and end stage renal disease, was on maintenance haemodialysis. She presented with altered mental status. Further investigations showed status epilepticus previously controlled with propofol and unspecified anti-epileptic agents. An underlying HE was confirmed and treatment with pulsed dose of IV methylprednisolone [Solu-medrol] 1g daily for 5 days was started. However, her condition did not improve. Thus, treatment failure of methylprednisolone was considered. The woman received treatment with prednisone after completion of methylprednisolone therapy and was successfully treated with therapeutic plasma exchange therapy. Attique HB, et al.
