Psychosis treated with risperidone: case report A 46-year-old man receiving methylprednisolone for optic neuritis developed psychosis, which was successfully treated with risperidone. The man presented with optic neuritis and started receiving systemic methylprednisolone 1000 mg/day. After the first dose, he developed agitation, anxiety and insomnia. On day 2, he had a dysphoric affect, became suspicious and had visual hallucinations. At his psychiatric examination, at the end of day 2, he had positive findings for delusions, hallucinations, hostility, hypervigilance, irritability, agitation, disorganised speech and thinking, insomnia, anorexia, decreased libido, poor reality testing and judgement, and had no insight into his condition. He was diagnosed with steroid-induced psychotic disorder. Methylprednisolone was discontinued on day 4, but his symptoms persisted. He began receiving risperidone 2 mg/day. After 2 weeks, he showed alleviation of symptoms and had no abnormalities in thought form and content and no sign of delusions within a month. Subsequently, his other psychotic symptoms, including hallucinations and agitation, also completely receded and he showed full recovery at the end of the month.
