Diabetes mellitus: case report A 29-year-old man developed diabetes mellitus during treatment with olanzapine for paranoid schizophrenia. The man, who had previously received risperidone and had been receiving olanzapine [dosage not stated] for 3 years, was hospitalised in a semi-comatose state after collapsing at home. He reported a 3-week history of general malaise, polyuria, polydypsia and weight loss; he had lost about 20kg of weight in the preceding 12 months through dieting and rigorous exercise and, at admission, he had a BMI of 24.8 kg/m2. On hospitalisation, his plasma glucose level was 68.8 mmol/L with 16.7% glycosylated haemoglobin. His urine was mildly positive for ketones; urinalysis showed evidence of acute kidney failure. He was diagnosed with new-onset diabetes mellitus and prerenal uraemia due to hyperglycaemic hyperosmolar syndrome. He was transferred to an ICU. Olanzapine was discontinued and, 2 weeks later, he was discharged while receiving oral antihyperglycaemics. [Patient outcome not stated.] Buchholz S, et al.
