Olanzapine Faecaloma: case report
A 70-year-old man developed faecaloma during treatment with olanzapine for chronic psychosis, and died eventually. The man, who had severe chronic psychosis, was admitted to hospital for acute abdominal pain. He had been receiving olanzapine [dosage and route not stated] for the chronic psychosis. He reported chronic constipation for the last 3 weeks. He also reported an increasing abdominal perimeter. He developed severe arterial hypotension. Subsequently, he required ICU admission for vasopressor support. Upon ICU admission, he had a severely distended abdomen with a diffuse collateral venous circulation. There was no clinical sign of ascites and spider angioma. Laboratory tests revealed hyperlactatemia (6.7 mmol/L) and acute kidney failure (creatinine 185 µmol/L) with anuria. An abdominal CT-scan revealed a severe faecal impaction with a marked backward compression of the kidneys together with a peritoneal effusion. There were no clinical signs of pneumoperitoneum, bowel pneumatosis and parietal thickening. Based on the clinical presentation and laboratory results, he was diagnosed with faecaloma [time to reaction onset not stated]. A conservative strategy without surgical extraction of the faecaloma was decided. However, he died due to multi-organ failure on day 2 of ICU admission. Logre E, et al. A fatal fecaloma.
