Lack of efficacy in critical condition: case report A 1-year-7-months-old girl exhibited lack of efficacy while receiving antibacterial therapy with amoxicillin for intermittent dry cough and high grade intermittent fever. The girl presented to a health care centre due to 3 weeks history of intermittent dry cough associated with high grade intermittent fever and loss of appetite. No history of contact with known pulmonary tuberculosis patient was reported. Initially, her treatment was started with amoxicillin syrup [dosage and route not stated]; however, after 7 days of treatment, no improvement was noted. Hence, she was referred to another hospital, but her parents preferred to take her home. After 2 weeks, she presented to the hospital with worsening symptoms. She was also noted as cachexic with evidence of severe acute malnutrition. On evaluation, Frank pus was noted with an impression of post pneumonic empyema thoracis, which was drained via insertion of a chest tube. She was then shifted to ICU. She received oxygen support, and wide spectrum antibiotics including ceftriaxone and metronidazole. Later, she was diagnosed with iatrogenic simple pneumopericardium with pneumothorax related to misplaced chest tube. She received conservative treatment and improved significantly. After significant improvement was noted her condition, she was discharged from the hospital in a stable condition. Mindaye ET, et al.
