Acute interstitial nephritis: case report A 16-year-old girl received a 6-day course of azithromycin 10 mg/kg/day (3g total) for bronchitis [route not stated]. Two weeks later, she presented with a 10-day history of vomiting, fatigue, malaise and abdominal pain. On admission, laboratory investigations revealed BUN and serum creatinine levels of 60 mg/dL and 2.6 mg/dL, respectively. Urinalysis showed 2+ protein, 10 WBC/hpf and eosinophils. Acute interstitial nephritis was diagnosed, and subsequently confirmed by renal biopsy. She received methylprednisolone for 3 days, followed by prednisone for 2 weeks. Two weeks later, laboratory tests revealed a serum creatinine level of 1 mg/dL and normal urinalysis findings. Renal function fully recovered after another 4 weeks. Persico C, et al.
