Coatings and oedema of oral mucous membranes and other toxicities in neonates: 3 case reports Three neonates developed coatings and oedema of their oral mucous membranes and other toxicities following chlorhexidine poisoning. At the age of 1–2 days, they were accidentally given 1mL of an oral chlorhexidine 20% solution instead of glucose 30%. A baby boy developed mild burns around his larynx. He also had an erythematous and swollen epiglottis. His upper oesophageal sphincter showed irritation, and his palatine arch had coating [time to reaction onset not stated]. He started recovering after symptomatic treatment. A baby girl developed swelling and coatings in her mouth in 8 hours. Swelling and coating of the epiglottis was seen upon oesophagoscopy, and ulceration was seen on both her uvula and palatine arch. In addition, large areas of her superficial gastric mucous membrane had peeled off. She started recovering after symptomatic treatment. Due to the possible development of an oesophageal stricture, she remains in follow-up. Another baby boy, who was 1-day-old at ingestion, aspirated some of the solution. He also developed breathing difficulties. Swelling was seen in his epiglottis and larynx. In addition, he had mucosal membrane swelling in his trachea with coatings down to bifurcation [time to reaction onset not stated]. He started recovering after symptomatic treatment. Author comment: "At concentrations above 20% [chlorhexidine] can be caustic. We describe three newborns who were accidentally given orally 20% of chlorhexidine". Nyman T, et al.
