Diltiazem Diffuse gingival enlargement in an elderly patient: case report An 80-year-old man developed diffuse gingival enlargement during treatment with diltiazem. The man, who had a history coronary artery disease and depression, presented with at least a 2-year history of extensive gingival hypertrophy and a 1-year history of pain upon mastication on the palatal gingiva of his maxillary anterior teeth. Even though he had tried regular use of antiseptic mouthwashes, frequent visits to the dentist for scaling and brushing his teeth twice daily, his gingival hypertrophy had not improved. His medications, which he had taken for more than 10 years, included diltiazem [dosage and route not stated; duration of treatment to reaction onset not clearly stated]. Significant diffuse soft tissue overgrowth was seen upon intraoral examination. The overgrowth affected both jaws, was firm in consistency and had a pink-to-red colour. The patient’s anticoagulant regimen was modified, and an incisional gingival biopsy was done. He was diagnosed with pharmacologically induced gingival hyperplasia.
