Lack of efficacy: case report In a retrospective study of 61 patients, who underwent endoscopic retrograde cholangiopancreatography (ERCP) between January 2007 and December 2017, one patient [age and sex not stated] was described, who exhibited lack of efficacy during treatment with epinephrine for ERCP-associated bleeding. The patient, who had a history of end-stage renal disease (ESRD), was on haemodialysis. The patient underwent ERCP for choledocholithiasis. The patient received sedation with midazolam and pethidine [meperidine hydrochloride], and unspecified protease inhibitors for prophylaxis against post-ERCP pancreatitis. The patient developed bleeding following ERCP. Epinephrine (1:10 000) solution was sprayed on the bleeding site with application of a balloon tamponade followed by administration of epinephrine injections. However, despite receiving multiple sessions of endoscopic injection therapy, including massive blood transfusion, intensive care, angiography and fully covered metal stent insertion, the patient eventually died due to bleeding.
