Visual hallucinations: case report A 58-year-old man developed visual hallucinations during treatment with levodopa for Parkinson’s disease. The man was admitted because of a 1-year history of progressive right-hand tremor with the slowness of movement. Before the admission, he had a significant loss of motivation and attention fluctuations. He also had cognition culminating during episodes of brief blank staring. One month prior to the presentation, he experienced neck and shoulder pain, along with unexpected episodes of temporary loss of consciousness and falls. He had a 10-year history of dream-enacting behavior and hyposmia, along with myocardial infarction, depression and generalised anxiety disorder. His brother had been diagnosed with dementia, while his father had Parkinson’s disease. Concomitantly, he had been receiving aspirin and bromazepam. Upon further evaluation, differential diagnosis as dementia, multiple system atrophy, dementia with Lewy bodies, atypical Parkinsonism and Parkinson’s disease dementia was made. Hence, to evaluate dopaminergic response, he was treated with levodopa 300mg daily [route not stated], along with carbidopa. His motor features improved. However, he developed visual hallucinations [time to reaction onset and outcome not stated]. Hence, the man’s levodopa dosage was decreased.
