Contraceptive failure: case report A 28-year-old woman conceived following failure of levonorgestrel-containing intrauterine device (IUD) contraceptive. The woman, who had given birth to a healthy female baby, presented to a clinic to discuss the possibilities for contraception at 4 weeks postpartum. She reported that she was breastfeeding her daughter. Anamnesis revealed that she had used an IUD previously for contraception. After a negative pregnancy test, a levonorgestrel-releasing IUD [Mirena] was placed at 6 weeks postpartum [dosage not stated]. After 6 months, she again visited the clinic due to complaint of increased difficulty in maintaining her milk supply. She additionally reported a positive result in a home pregnancy test. At the clinic, a urine pregnancy test was also found to be positive. Ultrasound revealed a single intrauterine pregnancy of 21 weeks. Ultrasound showed the location of IUD within the cervix and myometrium. After discussion about the risks, she chose to leave the intrauterine device in the same place.
