Ondansetron withdrawal Flushing and tremors in an elderly patient: case report A 67-year-old man with a brachium pontis arteriovenous malformation (AVM) developed flushing and tremors upon ondansetron withdrawal. The man presented with dizziness, extreme nausea and vomiting, and diplopia over 24 hours. He had mild right dysmetria and right sixth nerve palsy. A brain CT revealed a cerebellar haemorrhage and a fourth intraventricular haemorrhage. Brachium pontis AVM and anterior-inferior cerebellar arterial supply and superficial venous drainage was demonstrated by angiography. He was admitted for radiosurgical therapy and embolisation. During hospitalisation, he received continuous IV ondansetron 4mg every 6 hours, which controlled his nausea and vomiting. Four days later, ondansetron was discontinued and, less than 24 hours later, he experienced extreme flushing of his head and neck and developed a diffuse mild tremor, which was exacerbated with movement. Ondansetron was restarted and, after 20 minutes, the man’s tremors resolved and his flushing returned to his baseline appearance. Vadivelu S, et al.
