Lack of efficacy: case report A male neonate [exact age at the time of reaction onset not stated] exhibited lack of efficacy following treatment with phenobarbital for convulsions. The boy was born vaginally and had a birth weight of 2600g. He was discharged home one day after birth. As a tradition, he was applied topical henna three days after birth. Few hours later, he exhibited decreased spontaneous activity, cyanosis and respiratory distress. He was hospitalised in critical condition. Emergency tracheal intubation was performed for respiratory failure. He was treated with fresh frozen plasma. CT scan revealed subarachnoid haemorrhage. He was treated with unspecified IV fluids. Subsequently, he developed convulsions. Therefore, he was initiated on phenobarbital therapy [route and dosage not stated]. However, his convulsions did not stop. Therefore, additional treatment with phenytoin was initiated. The boy developed kidney failure, respiratory failure and haematological failure. The multiorgan failure was considered to be secondary to henna application. His treatment was continued for a long time due to the effect of the central nervous system and he was discharged after total 40 days of hospitalisation. Katar S, et al.
