Acute generalised exanthematous pustulosis: case report A 57-year-old man with seizure disorder caused by traumatic head injury developed acute generalised exanthematous pustulosis (AGEP) after receiving phenytoin. The man presented with a recent onset of multiple pustular lesions that began on the face and spread to his trunk and upper extremities 5 days after receiving a single dose of phenytoin 1g. At this time he was receiving lorazepam and levetiracetam. Physical examination revealed numerous 1- to 3-mm nonfollicular round pustules covering his entire face and 2- to 3-cm erythematous plaques covering his chest, back and upper extremities. Laboratory examinations revealed hypocalcaemia and leukocytosis. The man was treated with prednisone for 4 days. Within 48 hours of treatment, the patient showed improvement and after 4 days most of the pustules had ruptured. He was discharged on day 6 on a prednisone taper. Aziz N, et al.
