Susac’s syndrome: case report A 22-year-old female-male transgender patient developed Susac’s syndrome (SS) during treatment with testosterone. The transgender patient, who had been receiving testosterone [route and dosage not stated] for 3 years, presented with psychomotor slowing and behavioural changes. The patient also reported vertigo, presenting from 2 weeks. The patient was admitted. The patient showed short-term memory impairment, severe in-attention, gait instability, pyramidal symptoms and frontal release signs. A brain MRI showed signs suggestive of SS including snowball-like lesions in the corpus callosum and lesions appearing like string of pearls in internal capsule. Additionally, sensorineural hearing loss at low frequencies was noted on audiometry. The patient also showed branch retinal artery occlusion on fluorescein angiography. Owing to the MRI findings and the presence of sub-acute encephalopathy, retinal artery occlusion and hearing loss, a diagnosis of SS was confirmed [time to reaction onset not stated]. The patient was started on unspecified immunosuppressive therapy [outcome not stated].
