Evaluation of a thyroid nodule remains one of the most common, and yet one of the most challenging, problems facing a practicing endocrinologist. Difficulties and uncertainties persist, particularly in assessing the likelihood of malignancy and determining the most appropriate management strategy. Even as significant new information about the molecular markers in thyroid nodules has become available for clinical use, the final decision about the likelihood of malignancy, the extent of surgery (if any) and the nature of post-surgical surveillance and therapy ultimately rests with the clinician rather than with any particular test.
