Tocilizumab Exanthema: case report A 61-year-old man developed exanthema during treatment with tocilizumab for rheumatoid arthritis. The man had been receiving tocilizumab [dosage not stated] for two months before presenting with reddish papules on his upper extremities and trunk; the rash had started several weeks after the initiation of tocilizumab. Examination showed multiple papules scattered on his trunk, buttocks and upper limbs. Some had crust or scale on their surface. A biopsy from a non-crusty papule showed a perivascular infiltrate of lymphocytes intermingled with eosinophils in the upper-to-middle dermis and some lymphocytes had invaded into the epidermis. There were more CD4+ T cells than CD8+ T cells in the infiltrate. Tocilizumab was discontinued and, within 7 days, the rash gradually subsided with topical corticosteroid therapy. Yoshiki R, et al.
