Hypersensitivity: case report A 37-year-old woman developed a hypersensitivity reaction to voriconazole. The woman was admitted for induction chemotherapy to treat acute myeloid leukaemia. She was started on IV voriconazole [dosage not stated] for a fungal sinusitis. During the voriconazole infusion she developed anaphylaxis with generalised maculopapular rash and bronchospasm. Her medical history revealed she had experienced a penicillin allergy with cutaneous rash in childhood, but had subsequently received oral penicillin and amoxicillin as an adult with no complications. As she needed to received voriconazole or IV amphotericin for 3 months, oral desensitisation to voriconazole was conducted. Following desensitisation, she received voriconazole with no reaction. Author comment: "The Naranjo Score of 4 suggested that voriconazole was the possible cause of the adverse event". Zwiener RD, et al. Successful desensitization to voriconazole.
