American Heart Association Heart Profilers http://www.americanheart.org/presenter.jhtml?identifier=3000416 The American Heart Association is a nonprofit organization whose single mission is to “reduce disability and death from cardiovascular disease and stroke.” This link leads to the Heart Profilers section of the American Heart Association website and provides interactive tools to help users make decisions regarding their cardiovascular health care. The website requires the user to complete a 15- to 30-minute questionnaire for each disease process (eg, heart failure, atrial fibrillation) before generating a personalized “Treatment Options Report.” Another questionnaire generates a “Study Report,” which provides survival and treatment outcomes. Although the Heart Profiler is a free service, it does require registration (e-mail and password). The Heart Profiler also requires users to be knowledgeable about their own health history. The Heart Profilers website is sponsored by Kos Pharmaceuticals, Inc.
American College of Cardiology http://www.acc.org/qualityandscience/clinical/consensus/risk/dirIndex.htm The American College of Cardiology is the largest professional organization for cardiologists and allied health care professionals. Their mission is to “advocate for quality of cardiovascular care—through education, research promotion, development and application of standards and guidelines—and to influence health care policy.” This link leads to the current version (1999) of the American Heart Association and American College of Cardiology joint statement on the assessment of cardiovascular risk through the use of multiple risk-factor assessment equations. This website is a useful review for health care providers.
Seattle Heart Failure Model http://depts.washington.edu/shfm/ This website provides a heart failure prediction model that was developed by Levy, Associate Professor of Medicine and Cardiology at the University of Washington. After the user enters baseline characteristics, including medications and device therapies, the website generates 1-, 2-, and 5-year prediction models. The website also generates hypothetical risk models as therapies are added. The website requires Java support. A downloadable Palm (Palm, Inc., Sunnyvale, CA) version is available, but a Windows Mobile (Microsoft Corp., Redmond, WA) version is not available. The Seattle Heart Failure Model is one of the first heart failure prediction models to consider medications and device therapies. This website is sponsored in part by the University of Washington and by Medtronic, Inc. (Minneapolis, MN).
