Pancytopenia: case report A 22-year-old man developed pancytopenia after receiving azathioprine for Crohn’s disease. The man, who had a history of Crohn’s colitis and perianal fistulectomy, was admitted with complicated perianal fistulas. After a seton operation with abscess drainage, he started azathioprine 25mg, increased to 50mg after 2 weeks [route and frequency not stated]. He then presented again with myalgia and high fever [duration of treatment to reaction onset not clearly stated]. Analysis revealed the following values: WBC count 180/µL, platelets 48 000/µL, haemoglobin 6.6 g/dL, CRP 24.7 mg/L and ESR 55 mm/h. Pancytopenia and septicaemia were suspected, and azathioprine was withdrawn. The man received granulocyte colony-stimulating factors and antibacterials.
