Pancytopenia and neutropenic sepsis: case report A 17-year-old girl developed pancytopenia and neutropenic sepsis during treatment with azathioprine for Crohn’s disease. The girl, who had Crohn’s disease, started receiving azathioprine [dosage and route not stated]. After 3 weeks of azathioprine initiation, she presented pancytopenia and neutropenic sepsis. She also had alopecia [aetiology not stated]. Following investigations, pre-existing homozygous NUDT 15 variant and wild type TPMT genotype were noted. Therefore, the girl’s azathioprine therapy was stopped, and she started receiving unspecified broad spectrum antibiotics. Eventually, she showed clinical improvement. Subsequently, she died due to intestinal obstruction and unspecified postoperative complications. Author comment: "In this report, we present a case of Azathioprine toxicity with pancytopenia with severe sepsis in a young patient with homozygous NUDT 15 variant and wild type TPMT genotype."
