Hypersensitivity reactions: 10 case reports In a retrospective study involving 98 patients referred between 2015 and 2018 due to previous hypersensitivity to iodinated contrast media, 10 patients [ages and sexes not stated] were described, who developed hypersensitivity reactions dysphagia, urticaria, angioedema or cutaneous rash following drug provocation tests with iobitridol, iodixanol, iomeprol or iopromide [duration of treatments to reaction onsets and outcomes not stated; not all routes stated]. The patients, who had a history of immediate or delayed hypersensitivity to iodinated contrast media, were scheduled to undergo drug provocation tests (DPT) with the same or different contrast media. The patients who had shown immediate hypersensitivity reactions (3 patients) underwent DPT with IV infusion contrast media 5mL, 30mL and then 60mL, whereas the patients with previous delayed hypersensitivity (7 patients) underwent DPT with contrast agent administered in two sessions with 7–14 days interval; 5mL and 30mL in the first session and 30mL followed by 60mL in the second session. Amongst the 10 patients the first DPT was carried out with iomeprol (3 patients), iodixanol (3 patients), iopromide (3 patients) and iobitridol (1 patient). All the patients showed positive reactions with hypersensitivity presenting as dysphagia (1 patient), urticaria (2 patients), angioedema (2 patients), generalised or face angioedema (2 patients), angioedema and urticaria (1 patient), and cutaneous rash (2 patients). Subsequently, 7 patients underwent a second DPT with a different contrast agent than the first DPT; three patients received iodixanol, three patients received iopromide and the remaining one patient received iomeprol. All the patients showed negative reactions, except one patient who underwent DPT with iopromide and developed hypersensitivity presenting as angioedema. Meucci E, et al.
