Immediate hypersensitivity: 2 case reports Two patients developed immediate hypersensitivity reaction following administration of iodinated contrast media or iobitridol [dosages not stated] for computed tomography [patient outcomes not stated]. A 43-year-old woman received iodinated contrast media and subsequently developed pruriginous maculopapular eruption, cough, dyspnoea, tachycardia and hypotension 5–10 minutes after administration. She was treated with methylprednisolone and chlorpheniramine. Intradermal tests to iohexol and ioversol were found to be positive at 1:100 dilution. A 67-yerar-old man received iobitridol and subsequently developed facial angioedema, generalized erythema, angioedema of the tongue, cough, dyspnoea, cyanosis, and refractory hypotension five minutes later. He had a serum tryptase of 67.5 ng/mL. He was treated with epinephrine [adrenaline], methylprednisolone, chlorpheniramine and volume expanders. Intradermal tests to iobitridol were positive at 1:100 dilution. Author comment: "We presented two cases of IgE mediated anaphylactic reactions to [iodinate contrast media] diagnosed by skin tests."
