Platelet function disorder associated with mucocutaneous bleeding: case report A 44-year-old woman developed platelet function disorder associated with mucocutaneous bleeding during treatment with gabapentin for chronic neuropathy. The woman was referred to the Department of Haemostasis due to severe menorrhagia (current presentation). Eight years prior to the current presentation, she was diagnosed with invasive ductal carcinoma of right breast. She achieved remission following treatment with chemotherapy, hormone therapy and radiotherapy, but the clinical evolution was complicated with chronic neuropathy. Therefore, 2 years prior to the current presentation, venlafaxine was started. Nine months prior to the current presentation, gabapentin 1100 mg/day [route not stated] was added to the treatment along with ketamine injections (administered every 3 months). However, 1 month after the start of gabapentin (8 months prior to the current presentation), she developed mucocutaneous bleeding. The ISTH-BAT score was estimated as six. After the current presentation, physical examinations showed normal results. Laboratory tests showed: haemoglobin of 10.5 g/dL and ferritin of 9 µg/L with normal platelet count and mean platelet volume. No morphological abnormalities were noted on blood smear. Other blood parameters including prothrombin time, activated partial thromboplastin time and thrombin time were also noted as normal. Her fibrinogen and CRP levels were slightly high. Platelet aggregation test demonstrated no induction of aggregation by epinephrine and arachidonic acid. As the bleeding developed only after the initiation of gabapentin and there was no history was bleeding events, the woman’s gabapentin therapy was discontinued. Within 6 days, resolution of bleeding was noted with complete normalisation of platelet aggregation tests. Baglo T, et al.
