Deep hemorrhagic solitary rectal ulcer : case report A 76-year-old man developed a deep hemorrhagic solitary rectal ulcer (aggravation of previously existing superficial rectal lesion) following administration of ibuprofen for mild upper respiratory tract infection and flu-like symptoms. The man presented with intense anal discomfort and proctorrhagia to an ambulatory unit in February 2016. He mentioned self administration of 2 tablets of oral ibuprofen 600mg for mild upper respiratory tract infection and flu-like symptoms. Clinical investigations showed mild anaemia with haemoglobin level of 10 g/dL. His history was significant for prostate cancer; diagnosed in 2010 treated with orchiectomy and radiation therapy followed by hormone therapy. In December 2013, he had experienced moderate anal discomfort and pain at defaecation; although no rectal bleeding or abdominal pain was noted. However, the GI endoscopy had shown a solitary rectal superficial ulcerated lesion, multiple telangiectasia, small internal haemorrhoids and left colon diverticulosis. At current presentation, due to new onset symptoms, the GI endoscopy indicated much deeper rectal ulcer with stigmata of recent hemorrhage at the same spot of the previous superficial ulcerated lesion. Thus, deep hemorrhagic solitary rectal ulcer (aggravation of previously existing superficial rectal lesion) secondary to ibuprofen use was considered [duration of treatment to reaction onset not stated]. The man was asked to stop ibuprofen and he was treated with rifaximin and hyaluronic acid. He exhibited good clinical evolution with immediate remission of pain. The next day, bleeding also subsided. In May 2016, at the follow up, a lower GI endoscopy showed mucosal healing and only a linear non-malignant scar was noted. Botianu A-M, et al.
