Listeria monocytogenes meningitis: case report A 51-year-old woman developed Listeria monocytogenes meningitis while receiving infliximab. The woman, who had been recently diagnosed with Crohn’s disease, started receiving infusions of infliximab 5 mg/kg [frequency not stated]. Six days after her second infusion, she presented to an emergency department with fever and anal pain. Sigmoidoscopy revealed anal oedema and an orifice, with erythema extending from the sigmoid to descending colon. The woman was admitted, and received piperacillin/ tazobactam and metronidazole. The next day, she developed persistent headaches, mental confusion and projectile vomiting. Examination revealed photophobia and nuchal rigidity. CT revealed blood in the lateral ventricles, while a lumbar puncture showed yellow CSF. Grampositive bacilli were detected in blood cultures. She started receiving meropenem and ampicillin. Subsequent cultures identified L. monocytogenes. After 4 weeks of treatment, CT and CSF analysis was normal, and she was discharged asymptomatic. Rodrigues S, et al.
