Rebound deterioration in vision, optic disc swelling and retinal haemorrhage: case report A 49-year-old man developed rebound deterioration in vision with worsening bilateral optic disc swelling with new retinal haemorrhage following completion of treatment with methylprednisolone for optic neuritis and multiple sclerosis. The man, who had a history of type 2 insulin dependent diabetes with peripheral neuropathy, hypertension, hepatitis B infection and left calf schwannoma resection, admitted with 2 week old gait disturbance and 2 day old acute left eye monocular vision loss. Following ophthalmologic and neurologic examination, primary diagnosis was optic neuritis and multiple sclerosis. Radiography identified a spinal schwannoma. He was treated with IV methylprednisolone [dosage not stated] for 3 days. Following the completion of the 3 day methylprednisolone treatment, he experienced a rebound deterioration in vision with worsening bilateral optic disc swelling along with a new retinal haemorrhage. The man started receiving treatment with plasma exchange; however, he later developed a central line-related sepsis. Subsequently, he underwent successfully laminectomy for the removal of the spinal schwannoma [outcomes of reactions not stated]. Author comment: "After completing 3 day intravenous methylprednisolone regimen, the patient had rebound deterioration in vision with worsening bilateral optic disc swelling with new retinal hemorrhage and was commenced on plasma exchange with 5 treatments."
