Neutropenic side effect: case report A 23-year-old man developed neutropenic side effect during immunosuppressive treatment with mycophenolate mofetil and prednisolone. The man, who had biliary atresia, underwent Kasai procedure at the age of 6 weeks. Subsequently, he underwent liver transplantation at the age of 23 years. Post-operatively, he started receiving immunosuppressive treatment with mycophenolate mofetil and prednisolone [dosages and routes not stated], along with tacrolimus. However, he developed neutropenic side effect, which was attributed to mycophenolate mofetil and prednisolone [time to reaction onset not stated]. The man’s mycophenolate mofetil and prednisolone dosages were gradually tapered. The dosage of tacrolimus was adjusted accordingly by maintaining tacrolimus therapeutic level at 8–10 µg/L. Subsequently, the tacrolimus dosage was gradually increased. He developed acute diarrhoea and a low-grade fever (aetiology unknown). On the first day of diarrhoea, two doses of tacrolimus were omitted, and tacrolimus therapy was restarted on the next day. After receiving symptomatic therapy for diarrhoea, he recovered rapidly [outcome of ADR not stated]. Teng TS, et al. Effect of diarrhoea on Tacrolimus trough level in a post liver transplant patient.
