First report of pulmonary tumour: case report A 36-year-old woman developed a pulmonary tumour with elevated carcinoembryonic antigen (CEA) titre following chronic aspiration of propolis. At a follow-up for asthma, the woman reported a 6kg weight loss over 6 months. She had blood eosinophilia (2.34 x 109 U/L) and a serum CEA titre of 60.8 ng/mL. Chest xray and CT scan revealed a tumourous lesion at the left lower lobe of her lung and mediastinal lymph node enlargement. Left lower lobectomy and lymph node dissection were performed. There was a brown, jelly-like mass distal to the left lower lobe bronchus, which measured 4.5 x 2.2 x 2.0cm. The mass consisted of small fragmented foreign bodies, heavy eosinophils and fibrinopurulent exudates. The bronchial epithelium revealed goblet cell hyperplasia with eosinophil infiltration. The goblet cells were immunoreactive to CEA. There were scattered foci of foreign body aspiration throughout the lung parenchyma. All mediastinal and lobar lymph nodes showed reactive lymphoid hyperplasia with eosinophil infiltration. A review of the woman’s history revealed that she had smelled and inhaled liquid bee glue [dosage not stated] via her nasal mucosa for the previous 6 months, as an adjuvant treatment for asthma. Her CEA titre decreased to 14.2 ng/mL after 2 weeks and to 7.88 ng/mL after 6 months. Her eosinophil count decreased to 0.46 x 109 U/L. » Editorial comment: A search of AdisBase, Medline and Embase did not reveal any previous case reports of tumour associated with propolis. The WHO Adverse Drug Reactions database contained no reports of neoplasm associated with propolis.
