Anaphylaxis in an infant: case report Following diagnosis of optic nerve glioma, a 1-year-old girl was scheduled to receive 1 year of chemotherapy comprising vincristine and IV carboplatin 300 mg/m2 given as a 1-hour infusion twice every 4 weeks. In cycle 7, she experienced an anaphylactic reaction with pruritus, maculopapular rash, diaphoresis, respiratory distress, anxiety and elevated HR. She was immediately given epinephrine [adrenaline], hydrocortisone and diphenhydramine which alleviated her symptoms. A skin test for carboplatin was positive. One week after her first hypersensitivity response, a rechallenge with carboplatin was performed and her anaphylactic symptoms recurred. Based on the need to continue carboplatin chemotherapy, a desensitisation protocol was undertaken consisting of a continuous and progressive infusion of diluted carboplatin. The procedure was performed prior to each cycle and she was able to complete her scheduled chemotherapy.
