Hypersensitivity: case report A 61-year-old man developed hypersensitivity after receiving a single dose of cefepime. The man, who had multiple myeloma, was hospitalised because of a 2-week history of fever that was unresponsive to treatment with ciprofloxacin and amoxicillin/clavulanic acid. He started receiving empirical IV cefepime 2 g/day. Five minutes after the first dose, he developed generalised pruritus and urticaria, wheezing and dyspnoea. The man received IV methylprednisolone and dexchlorpheniramine, and recovered quickly. He had tolerated treatment with cefepime 2 years earlier. The man underwent prick and intradermal skin testing 4 weeks later. Results were positive for cefepime, with a 9 × 10mm wheal. He tolerated ceftazidime and amoxicillin/ clavulanic acid at therapeutic dosages.
