Systemic allergic dermatitis: case report A 62-year-old woman developed systemic allergic dermatitis during treatment with gentamicin for thermal burn. The woman, who had superficial and partial thickness thermal burn on her left leg and foot following an accidental contact with boiling water, started receiving treatment with topical gentamicin 0.1% cream and chloramphenicol/ collagenase [Iruxol] ointment. Following 3 weeks, the burn wound worsened and the perilesional eczematisation was noted. Subsequently, an itchy eczema developed on her scalp and the extensor part of her limbs and hands. The woman’s treatment with gentamicin and chloramphenicol/collagenase was discontinued. She was treated with betamethasone and fusidic acid cream. Patch test was performed with neomycin sulfate and with all the topical products used, which were applied on the upper part of her back. The readings on day 2, 4, and 7 revealed a positive reaction only to gentamicin. During patch testing, a widespread flare of itchy eczematous dermatitis developed on whole body except the face. As a result, unspecified corticosteroids were prescribed and an eczema along with burn wound resolved. Six weeks later, a positive semiopen test with 25% gentamicin pet. on the right arm confirmed the diagnosis of systemic allergic dermatitis due to gentamicin.
