Isotretinoin Acute pancreatitis: case report A 19-year-old woman developed acute pancreatitis during treatment with isotretinoin for inflammatory acne. The woman, who had a baseline triglyceride level of 151 mg/dL (normal < 150), started receiving isotretinoin 60 mg/day; concomitant medications included sertaline and ethinylestradiol. Two months later, she presented with a 2-week history of recurrent, sharp, nonradiating, postprandial pain in her upper abdomen that included several episodes of vomiting. She experienced dryness of the skin, especially around her lips, a decreased appetite, increased agitation, feelings of sadness and mild anxiety. On examination, she had a mild right epigastric tenderness without guarding or rebound. Laboratory investigations revealed a triglyceride level of 351 mg/dL and a lipase level of 95 U/L (16–63). The next day, her lipase level increased to 109 U/L. Acute pancreatitis secondary to isotretinoin was diagnosed. Isotretinoin was immediately discontinued. [Patient outcome not stated]. Greene JP.
