Propofol infusion syndrome in paediatric patients: 2 case reports During heart surgery, two girls, aged 12 and 16 years, received infusions of propofol [dosages not clearly stated] for general anaesthesia induction and maintenance. After surgery, they were transferred to an ICU and received continuous IV propofol for sedation. Both girls then developed progressive metabolic acidosis [times to reaction onsets not clearly stated]. Despite administration of sodium bicarbonate, the girls’ lactate levels continued to increase and reached maximum levels of 9.3 mmol/L and 14.7 mmol/L , respectively. Propofol infusion syndrome was considered and propofol, which had been infused at < 3 mg/kg/h, was discontinued after 15 and 8 hours in the two girls, respectively. Metabolic acidosis subsequently gradually resolved and both girls recovered. Author comment: "Our hypothesis that the observed lactic acidosis indicates an early stage of [propofol infusion syndrome] is sustained by some arguments, especially including: absence of other usual causes for lactic acidosis, close association with propofol infusion, and quick reversibility of acidosis without specific intervention upon cessation of propofol infusion."
