Purified talc/viscum album Acute respiratory distress syndrome and pneumonitis: 3 case report

In a retrospective study of 56 patients with malignant pleural effusion who had undergone tube thoracostomy followed by chemical pleurodesis between January 2003 and December 2017, three patients [ages and sexes not stated] were described, who developed acute respiratory distress syndrome or pneumonitis following treatment with purified-talc or viscum-album as a sclerosing agent. The patients, who had malignant pleural effusion, underwent tube thoracostomy followed by intrapleural injection of purified-talc [Steritalc] 2g diluted in sterile sodium-chloride [saline] in 2/3 patients and intrapleural injection of viscum-album [ABNOVA Viscum] 100mg mixed with sodium-chloride [saline] in 1/3 patients. All three patients were locally anesthetised by lidocaine during the procedure. Following the pleurodesis, the 2 patients who received purified-talc as sclerosing agents developed acute respiratory distress syndrome (1 patient) and pneumonitis (1 patient) and the patient who received viscum-album as a sclerosing agent developed acute respiratory distress (1 patient) [durations of treatments to reactions onsets and outcomes not stated]. Chang YJ, et al. Viscum pleurodesis is as effective as talc pleurodesis and tends to have less adverse effect. Supportive Care in Cancer 28: 5463-5467, No. 11, 2020.
