Sertraline REM sleep behavior disorder: case report A 39-year-old woman developed REM sleep behavior disorder during treatment with sertraline for depression/ anxiety and post-traumatic stress disorder symptoms (PTSD). This case was reported to FDA’s adverse event reporting system (FAERS) as a voluntary report. The woman had a significant medical history of spinal cord injury from a gunshot wound (19 years ago), which resulted to PTSD, paraplegia and chronic sleep problem. At the time of initial evaluation, she was receiving amitriptyline for neuropathic pain and was started on sertraline 12.5 mg for anxiety symptoms [route not stated]. Following the treatment with sertraline, a mild improvement in anxiety symptoms was noted. The dose of sertraline was increased to 25mg. She also reported mild symptoms of crawling feeling in leg, leg kicking and sometimes sleep talking. However, polysomnography for any obstructive sleep apnea was negative. Her dose of sertraline was gradually increased to 150mg and further to 200mg for better control of depression/anxiety and PTSD symptoms. At the dose of 200mg, she experienced worsening of her night time sleep behavior. Additionally, her mother noticed sleepwalking episode and doing things that she (the patient) had no recollection in the morning including eating from the refrigerator and trying to take a bath [duration of treatment to reaction onset not stated]. The woman decreased the dose of sertraline to 12.5mg and her symptoms of parasomnia resolved. However, worsening of anxiety was noted. Hence, she started receiving escitalopram and sertraline was discontinued. At the time of report, she did not experience any sleep related behavior.
