Sexual dysfunction: case report An approximately 21-year-old man developed sexual dysfunction following withdrawal of sertraline. The man had a major depressive episode at the age of 21 years, for which he started receiving sertraline 100 mg/day [route not stated]. Thereafter, sertraline was gradually tapered off. However, he developed new symptoms, which were presented as: erectile dysfunction, emotional flattening, reduced creative capacity/abstraction/imagination, premature ejaculation, absence of libido and sexual pleasure, tactile anaesthesia and temperature in the genital area and a sense of detachment and alienation. These symptoms remained unchanged for further 4 years. The man received amisulpride, aripiprazole, buspirone, bupropion and pramipexole; however, benefits were not seen. Evaluation based on the structured clinical interview for Diagnostic and Statistical Manual of Mental Disorders (DSM-5) showed current major depressive disorder, generalised anxiety disorder, persistent depressive disorder and obsessive-compulsive disorder; while the Diagnostic Criteria for Psychosomatic Research-Revised revealed allostatic overload, persistent somatisation, hypochondria, somatic symptoms secondary to a psychiatric disorder, demoralisation. Based upon evaluation of the Diagnostic clinical Interview for Drug Withdrawal-1, he was diagnosed with new withdrawal symptoms manifesting as sexual dysfunction. It was thus determined that he had developed symptoms of post-selective serotonin reuptake inhibitor sexual dysfunction, which were considered to have been induced by the withdrawal of sertraline [time to reaction onset and outcome not stated]. Author comment: "[Post-selective serotonin reuptake inhibitor sexual dysfunction] symptoms are part of a broader syndrome which was diagnosed via the [Diagnostic clinical Interview for Drug Withdrawal 1] and is named new withdrawal symptoms.
