Gastrointestinal bleeding or epistaxis in children: 2 case reports Two children developed gastrointestinal bleeding or epistaxis, after treatment with risperidone. A 6-year-old boy with borderline mental capacity and disruptive behaviour disorder was initiated on risperidone 0.25mg twice daily. Six days into treatment, his mother noticed blood and mucus in his faeces. Three days later, he tested positive for occult blood. Risperidone was discontinued, but was later reinitiated using the same dosage. Over the subsequent 2 weeks, he twice tested positive for occult blood. A possible adverse reaction to risperidone was suspected and the drug was ceased. During 1 month of follow-up, he tested negative for occult blood on two separate occasions. An 11-year-old boy had a history of spontaneous episodic epistaxis, but had not experienced epistaxis for 4 years. He was diagnosed with attention-deficit hyperactivity disorder and oppositional defiant disorder, and started on risperidone 0.5mg daily at bedtime. On the fifth day of treatment, he had an episode of epistaxis during sleep, about 2 hours after taking his medication. Two days later, he had another episode during the day. Risperidone was ceased, and over 2 weeks he did not experience further episodes. The drug was then recommenced at the same dosage in the morning, and 2 days later he experienced epistaxis, 1 hour after taking his medication. Risperidone was withdrawn and over the next 2 months there were no further episodes. Author comment: "[I]t is difficult to state that risperidone definitely is the cause of the bleeding in both cases. However, the chronological sequence and response to drug discontinuation in the absence of identifiable medical causes are suggestive of possible causality." Coskun M, et al.
