Risperidone Intraoperative floppy iris syndrome: 2 case reports Two patients developed intraoperative floppy iris syndrome (IFIS) during treatment with risperidone [routes and dosages not stated]. A 50-year-old man with well-controlled type 2 diabetes mellitus was scheduled for cataract surgery. He had been taking risperidone for schizophrenia for the past 6 years. Preoperative pupil dilation was poor despite administration of mydriatics. During surgery, classic iris flaccidity and billowing of IFIS and progressive miosis were observed. The procedure was completed uneventfully. Four months later, he underwent surgery on his second eye, which exhibited a similar behaviour. A 76-year-old woman was receiving long-term risperidone therapy for bipolar disorder when she presented for cataract surgery [duration of therapy not stated]. Poor pupil dilation was noted preoperatively, despite mydriatics. Features of IFIS were noted during surgery, with undulating iris flaccidity and progressive miosis.
