Painless vision loss, intraretinal haemorrhage and valsalva maculopathy: case report A 38-year-old man developed painless vision loss, intraretinal haemorrhage and valsalva maculopathy following sildenafil overdose. The man presented to the ophthalmic emergency room with painless vision loss in the left eye. He reported that he observed the vision loss in the morning following having repeated, strenuous sexual activity during the previous night. Also, he received multiple oral sildenafil tablets. Examinations revealed best-corrected decimal visual acuity was 0.5 in the left eye (OS) and 1.0 in the right eye (OD). Anterior segment was found to be unremarkable in both eyes along with normal intraocular pressure. Funduscopic examination showed an oval macular hemorrhage OS and normal findings OD. Optical coherence tomography showed the presence of an intraretinal macular hemorrhage. Fluorescence angiography and ultrasound of the carotid artery were unremarkable. Blood count and laboratory examination including thrombophilia and rheumatology screenings did not reveal abnormal findings. Based on these findings, he was diagnosed with valsalva maculopathy [duration of treatment to reaction onset not stated]. The man was monitored closely. Following 5 months at follow-up examination, the macular haemorrhage had almost complete resolution and his visual acuity improved to 1.0. Steinemann A, et al.
