Warfarin Iliacus haematoma leading to femoral neuropathy: case report A 63-year-old man, who had a history of atrial fibrillation and aortic valve replacement, developed an iliacus haematoma leading to femoral neuropathy during thromboprophylactic treatment with warfarin. The man, who was receiving warfarin [dosage and duration of treatment not stated], presented with a 2-day history of sudden-onset acute right lower limb paraesthesia and weakness. His hip was tender on passive extension and his L1–L4 dermatomes were numb; he could not tolerate motor testing of his hip. Laboratory investigations revealed an abnormal coagulation profile with an INR of 7.5; on ultrasound, possible fluid was detected in his fascial planes over the anterolateral aspect of his proximal right thigh. An MRI scan confirmed the diagnosis; a homogenous mass was observed within his right iliacus, anteriorly displacing the right psoas. The man received fresh frozen plasma and conservative management.
