Auditory and visual hallucinations, and psychosis: case report A 37-year-old woman developed auditory and visual hallucinations, and psychosis during treatment with hydroxychloroquine for discoid lupus erythematosus (DLE). The woman presented to the dermatology clinic with a complaint of a red lesion on her left cheek. Following investigations, she was diagnosed with DLE. Topical clobetasol and pimecrolimus were prescribed, and she was recommended to use unspecified sunscreen. One month following the treatment, she returned with aggravation of the lesion. Therefore, hydroxychloroquine 200mg daily [route not stated] was added to the treatment regimen. One week after starting hydroxychloroquine, she returned to the clinic with the complaint of auditory and visual hallucinations, along with psychosis, presenting as nightmares and occasional decrease in consciousness level. A neurology consultation was carried out to rule out organic aetiologies, which confirmed that she had no neurologic lesions. Later, a psychiatric consultation was requested. According to the clinical data and lack of a history of psychiatric disorders, hydroxychloroquine was recommended to be discontinued. Hydroxychloroquine was replaced with monthly intralesional triamcinolone. The hallucinations and other symptoms resolved within 1-week after discontinuation of hydroxychloroquine.
