Thiamine metabolism dysfunction syndrome-II: case report In a study, a 2-year 5-month-old girl developed thiamine metabolism dysfunction syndrome-II (THMD2) following vaccination with measles vaccine. The girl was vaccinated with measles vaccine [dosage and route not stated]; however, approximately half a month after the vaccination, she developed fever, followed by general muscle weakness on the second day of fever. Thereafter, she had inability to walk, week cry, dysarthria, dystonia, ptosis and positive bilateral Babinski signs. She was then hospitalised as suspected encephalitis to a local hospital. The girl received unspecified treatment and fully recovered. However, at the age of 4 years 2 months, she developed second attack. A week later, she achieved near complete recovery. At the age of 4 years and 3 months, she presented with a recurrent feeling of numbness in her hands and feet. A cranial MRI revealed abnormal signals in the bilateral caudate nucleus, putamen and cortex. She was found to have Leigh syndrome due to THMD2. Later on, she was treated with thiamine and biotin, and a significant improvement was observed. Her feeling of numbness disappeared gradually. Noval mutations were found in SLC19A3, which was associated with THMD2. For her THMD2, vaccination with measles vaccine was considered as contributing factor. Li D, et al.
