Iodine 131 Graves ophthalmopathy: case report A 49-year-old woman, who had a history of total thyroidectomy and radiotherapy, received 2 days of thyrotropin-α, followed by iodine 131 100mCi in August 2006 for recurrent thyroid cancer. She had also received the same dose of iodine 131 in January 2001. She underwent external beam radiotherapy (3000 rads) in November 2006 for osseous metastases, and zoledronic acid was started. Iodine 131 150 mCi was administered in March 2007. Several weeks prior to this treatment, periocular swelling and excessive tearing had developed. An MRI scan revealed bilateral proptosis and enlargement of her extraocular muscles, indicative of Grave’s ophthalmopathy. Ophthalmologic examination showed bilateral periocular oedema, mild bilateral exophthalmos and chemosis, and limited repulsion of palpation. Thyroperoxidase and thyroid-stimulating immunoglobulins were also elevated [patient outcome not stated].
