Fatal pneumonitis in an elderly patient: case report An 81-year-old man with small-cell lung cancer received four cycles of chemotherapy with cisplatin and etoposide before commencing irinotecan [dosage not stated]. He was hospitalised with dyspnoea after three infusions, and ground-glass opacities were observed bilaterally on CT scan. Antibacterials and corticosteroids were started, with a rapid recovery. Irinotecan was resumed, but he was hospitalised with respiratory failure 2 weeks after his fourth infusion. Mechanical ventilation was initiated, and chest Xray showed diffuse interstitial infiltrates and dense infiltrates on his right side. He subsequently died, despite extensive treatment.
