Fixed drug eruption: case report A 28-year-old man developed a fixed drug eruption following a single dose of naproxen for a headache. The man was admitted with a erythematous, inflammatory lesion on his lip, extending to his left shoulder and the posterior region of his neck. He had developed a pruritic, painful skin lesion approximately 3 hours after a single dose of naproxen 550mg [route not stated]. He reported having experienced similar symptoms whenever taking a naproxen-containing analgesic after sport activity. His symptoms would disappear after discontinuation of the analgesic, but a scar had formed after resolution of the lesions on his left shoulder. On examination, a 5 × 5cm, erythematous, hyperpigmented plaque lesion was evident on his left shoulder. Multiple, well-defined, circular, oedematous, erythematous plaque lesions were also observed orally, on his neck, and on the left palmar region. Histological examination of the left shoulder lesion revealed hydrophobic degeneration in the basal lamina, pigment incontinence, and chronic inflammatory cell infiltration of leucocytes and lymphocytes around vascular structures. A fixed drug eruption was diagnosed. Systemic corticosteroid and oral antihistamine therapy was initiated. The eruption regressed by day 2 of treatment. The man was notified about naproxen use.
