Coma: case report A 34-year-old woman with chronic schizophrenia went into a deep coma after a quetiapine overdose. The woman, who had a history of drug addiction and HIV/HCV co-infection, attempted suicide by ingesting 120 controlled-release quetiapine 300mg tablets (total dose of 36g). She was found in a lethargic state [time to reaction onset not stated], which quickly deteriorated. Upon admission to hospital, she was in a deep coma and only responded to deep painful stimuli; she had a Glasgow Coma Scale score of 9 and stable vital signs. She had a partial pressure of carbon dioxide of 92mm Hg and a bicarbonate level of 24 mmol/L, and was intubated. Negative results for opioids, cocaine, amphetamine, methadone and tetrahydrocannabinol were obtained from urinalysis, and immunoassay screening was positive for quetiapine. A diagnosis of drug toxicity was made. The woman received supportive care and, in about 36 hours, spontaneous breathing was restored. Her antiretroviral therapy was restarted, and she also received diazepam after experiencing some psychotic symptoms. The woman was then transferred to another hospital where she started retreatment with quetiapine. Eight days later she was discharged due to refusal to continue hospitalisation, and a week later she reported fatigue, somnolence and dyspnoea which resolved over the next few days. At follow-up after 4 months, she was receiving quetiapine and lorazepam with no significant abnormal clinical findings. Capuano A, et al.
