Quetiapine Photopsia: case report A 22-year-old man developed photopsia during treatment with quetiapine for schizoaffective disorder, bipolar type. The man, who was stabilised on treatment with quetiapine at a dosage of 900mg every night, lithium, and clonazepam over a 3-month hospitalisation period [duration of treatment not clearly stated], had his quetiapine dose increased to 1000mg every night. The next day, he developed "intermittent flashes of a horizontal streak of silver light" in both eyes that were accompanied by mild eye discomfort. The flashes occurred intermittently during the day, were worse in bright light, and lasted up to 15 minutes. He reported the symptoms 13 days later and quetiapine was reduced to 900mg every night before bed due to photopsia. Following the dose adjustment, the light flash frequency rapidly decreased, and his eye discomfort dissipated. Within several days, his photopsia resolved. Author comment: "We believe that the symptoms experienced by this patient were consistent with photopsia, and that they were most likely related to the increase in the dose of quetiapine."
